Responsibilities / Accountabilities:
· Commitment to the Mission and Core Values of Peace Care.
· Participate in care plan patient/family meetings.
· Participate in interdisciplinary team rounds.
· Conduct a complete bio-psychospiritual patient/family assessments.
· Collaborate with the care team to develop a spiritual care plan as an essential component of the overall plan of care.
· Document assessment, interventions, and outcomes in the patient electronic health record, preferably as part of an interdisciplinary plan of care note.
· Oversee activities of pastoral care volunteers and visiting clergy, including the mass schedule and special prayer services and liturgies.
· Coordinate the implementation of quality improvement (QAPI) projects through direct collaboration with other departments.
Experience Requirements:
· Skilled Nursing Facility or Hospice experience required.
· Point Click Care experience strongly preferred.
· Excellent communications skills.
· Attention to detail and follow through a must.
· Able to work collaboratively with others.
· Professional demeanor and ability to interact with residents and family members.
Qualifications:
· Accredited master’s degree in theological studies or its equivalent.
· Minimum 3-5 years Pastoral care or Chaplain experience in a healthcare setting with ability to develop Pastoral Care programming.
· Must be capable of maintaining regular attendance during normal business hours.
